The type of personal information I collect of my patients:

I currently collect and process the following information:

• Personal identifiers, contacts and characteristics (for example, name and contact details). I use a third-party provider to allow patients to make or rearrange appointments.

• Information related to your past and medical history, including test results or reports you share.

• Details about your lifestyle and personal situation.

• I keep a record of your treatments, details of progress of your case and any advice given to you for a period of seven years following your most recent appointment (or once you have reached age 25 if longer). 

• ​I keep a permanent attendance register which records all appointments for patients attending the clinic.

• In the event of an adverse incident occurring to any of mypatients, I report the matter to our professional body and insurance company.

How I get the personal information and why I have it

Most of the personal information I process is provided to me directly by you for one of the following reasons:

• I need to collect personal information about your health in order to provide you with the best possible treatment by planning and tracking your treatment.

• I have a legitimate interest in collecting that information, to ensure I can practice acupuncture effectively and safely.

• Records are kept of your contact information so I can contact you to update you on matters related to your treatment or remind- you of appointment times.

Personal Data held for subscribers to my newsletter:

I maintain and use records of subscribers to the newsletter with the express consent of the subscriber for marketing purposes only. I will only hold your first name, last name and email address. This data will only be used for the purpose of sending you the newsletter, which will be distributed using a third-party provider.

Personal Data held for website use:​

For anyone who visits my website, I use a third-party service to collect standard internet log information and details of visitor behavioural patterns. I collect such data to monitor the number of visitors to the website and how the website it used. This information is processed in a way which does not identify any individual. I do not make, or allow the third-party provider, to make any attempt to find out the identities of those visiting the website.

I may use website cookies to improve the user experience of the website by enabling the website to ‘remember’ users, either for the duration of your visit (using a session cookie) or for repeat visits (using a persistent cookie).

Website search queries and results are logged anonymously to help improve the website and search functionality. No data which identifies an individual is collected by me or any third-party.

How I store your personal information

Your information is securely stored on a secured onedrive on a machine that is password protected.

I have a legal obligation to retain your records for 8 years after your most recent appointment (or after you have reached age 25, if this is longer), after which time the records are deleted. Any paper notes will be uploaded online and shredded immediately.

Your information will not be shared with third parties unless I have a legal obligation. Under the UK General Data Protection Regulation (UK GDPR), the lawful bases I rely on for processing this information are:

Your Personal Data will be treated as strictly confidential and will only be shared when necessary with:​​

• the police of a court of law, if necessary to comply with a legal obligation;
